Tell us your name, age, what city you’re based in, and your artist/band name. Please write a short bio about yourself, and tell us what you would want from a record deal with Cool Dog Records. Provide links to social media (if available).

We will only listen to links (i.e. YouTube or Bandcamp). We will not open any files.
